General Info from DHHS 

Step 1 - See if you are on Central Registry 

DHHS office addresses 

Step 2 - If on Central Registry, obtain a copy of the DHHS file(s). A written request must be mailed to your local DHHS office with a copy of your photo id. 

Step 3 - Seek expungement

Step 4 - If denied, seek an administrative review
