Correcting Credit Reports - Equifax

To check your credit history through Equifax and file a dispute on-line, you can go to their website.

This item [for instance: retailer account at ABC Department Store] is inaccurate [or incomplete] because [describe in detail what is inaccurate or incomplete and why]. I am requesting that this item be removed [or request another specific change to correct the information].

Enclosed is documentation supporting my request: [describe the documents you’re sending, for instance: my credit report, with the disputed items circled.] 

Proof of Identity/Verification

☐ All my previous names.

☐ All my previous addresses.

☐ Enclosed copy of Michigan Drivers/Identification Card.

☐ Enclosed copy of utility bill, bank or insurance statement, etc.

☐ Completed Equifax Dispute Form (includes SS#, DOB, etc.)

☐ Copy of Equifax Credit Report (if applicable).
